Tilt test (passive orthostatic test) involves a rapid, passive, and controlled change from a horizontal to a vertical body position using special equipment — a tilt table (verticalization table).

The tilt test helps in diagnosing:

Syncopal states (fainting or near-fainting / syncope or presyncope)

Arrhythmia

Heart disease

Neurological disorders

Single syncopal episodes (fainting) of unclear etiology in patients with a high degree of risk (e.g., real or potential risk of trauma, occupational aspects)

Recurrent syncope in patients without heart disease

How is a passive orthostatic test performed?

When the patient changes position, blood is deposited in the lower part of the body and the pressure in the right heart chambers decreases. In patients with neurocardiogenic syncope, this can lead to sudden hypotension and loss of consciousness.

During the diagnostic procedure, blood pressure and ECG are monitored. Decoding of ECG data and the time elapsed before loss of consciousness provide information about the state of the nervous, cardiovascular and endocrine systems. The results of the tilt test are crucial for confirming and clarifying the causes of syncope and contribute to the correct choice of treatment.

The procedure consists of several stages:

  1. Preparation: the patient is placed on a tilt table, a blood pressure cuff is applied, and an ECG device is connected.
  2. Baseline measurements: the patient lies quietly for 5–20 minutes, after which baseline heart rate and blood pressure are recorded.
  3. Tilt: the table is quickly moved to an angle of 60°–80°. The patient remains in this position for 20–45 minutes.
  4. Provocation (if needed): if the test is inconclusive, medications (e.g., nitroglycerin) may be administered to provoke a response.
  5. Completion: the patient is returned to a horizontal position if they feel weak, begin to lose consciousness, or after the allotted time has passed.

Importantly:

  • The examination is performed on an empty stomach.
  • 2 days before the examination, the patient should stop taking medications that affect the heart rhythm and heart rate (the decision is made by the attending physician).
